Working with Care Managers

We work closely in partnership with care managers and purchasers to develop our services, which will not only meet the current local needs and complement existing service provision, but will deliver improved support to our service users and meet the purchasers ongoing needs.

Managing Referrals
Our specialist placements team act promptly on all referrals received and will immediately identify the most appropriate services for the individual and arrange an introductory visit.

In some instances, a referral may be received directly from facilities and relatives, however the placements team also ensure that a local social services or healthcare trust care manager is involved from the onset to support these enquiries.

We work rigorously on identifying the most appropriate home to suit the individual needs and wants of the service user and strive for service user compatibility. We work closely with each individual and their support networks to create a bespoke programme that meets all their requirements, in the specific home of their choosing.

Ultimately, we tailor the support needs of each individual to ensure their comfort, health, safety and aspirations are met and quality of life is high.
